Licensed Practical Nurse (LPN) New

A nonprofit organization on Long Island wants to add a Licensed Practical Nurse (LPN) at one of their residential facilities in Suffolk County. In this role, the Licensed Practical Nurse (LPN) will be responsible for supporting the Day & Residential programs.

About the Opportunity:

• Schedule: Monday to Friday (No weekends or holidays)
• Hours: 7:30am to 4pm or 8:30am to 5pm (very flexible)
• Patient Population: Adult ID/DD OPWDD
• Patient Volume: Caseload of 16
• Setting: Residential facility

Responsibilities:

• Participate in written nursing plans of care for caseload with RN Supervisor
• Participate in planning and implementation of training of DDI personnel
• Maintain medical record for caseload
• Ensure all medications are administered and stored. as necessary
• Ensure medical follow through on all medical appointments, including any new orders and follow appointments needed
• Act as a liaison with the Pharmacy team in ensuring medications are provided to residence
• Ensure compliance with OPWDD regulatory standards and regulations

Qualifications:

• Graduate of an accredited School of Nursing
• NYS Licensed Practical Nurse (LPN)
• Solid assessment, clinical, and documentation skills
• Patient oriented
• Great interpersonal skills
• Excellent communication skills (written and verbal)
• Strong attention to detail
• Highly organized

Desired Skills:

• Experience working with the DD / ID populations
• Knowledge of OPWDD regulations
